Applications are invited, on the prescribed application form, from eligible candidates for admission to the following undergraduate programs (Fall 2020 Entry), to be offered at the UHS Jinnah Campus, Kala Shah Kaku:

Eligibility Criteria
Punjab-domiciled candidates who have
passed FSc. (Pre-Medical) or
equivalent examination with minimum
(660/1100) marks.
Punjab-domiciled candidates who have
passed F.Sc. (Pre-Medical) or
equivalent examinaüon with minimum
(550/1100) marks.
Punjab-domiciled candidates who have
passed F.Sc/F.AJequivalent examination
With minimum (550/1100) marks.
HOW TO APPLY.
1. The application process shall consist of two mandatory phases:
(i) online submission
(ii) Manual Submission
2. The candidate must till prescribed online application form available at UHS website www.uhs_edu.pk.
3. Before applying online, the candidate must deposit a processing fee of Rs.2000/- (Non-refundable) through
bank challan at National Bank of Pakistan, Sheikh Zayed Hospital Branch Lahore (NC # 3164498127) or get a
Pay Order,'Demand Draft of the same amount in the name of the University of Health Sciences Lahore. The
candidate shall enter the number of his/her bank challan/DD/PO in his/her online application form.
4. Applicants must till the storm carefully. Heme is required to complete the application by entering the correct
information. After completely filling the application, the candidate must review the information entered by
him/her. Before submission, the candidate will be able to edit his/her information. Once the application is submitted-
ted, the candidate will not be allowed to edit it.
5. Atter submission, the candidate iS required to geta print Of the online tilled application form. The printout.
application form, along with the attested copies of documents and certificates (as listed in the application form),
original bank cnallan/0D/PO, and recent passport size colored photographs, must reach the Office ot the Director
Admin & Coord, University of Health Sciences, Khayaban-e-Jamia Punjab, Lahore-Pakistan, by post or by hand,
on or before Wednesday, 28th oct0ber, 2020, by 05:00 PM.
6. It is mandatory for the candidate to complete both phases of application submission, i.e., online and manual.
Mere submission of online applications shall not be considered valid tor the candidature of the applicant.

Applications received after the due date or With incomplete information shall be rejected.
Admission shall be granted on open merit basis only.
Those having foreign qualifications must submit an equivalence certificate issued by IBCC.
